Praxis III Instructions, Application and Forms

Teachers with a three-year Initial License will take Praxis III during the second semester of the first year of teaching. Teachers with a Non-Traditional Provisional License will take Praxis III during the second semester of the second year of teaching.

Note: Praxis II PLT or the subject specific Pedagogy must be passed before taking Praxis III.

Step 1: Complete On-line Candidate Profile/Application

Note: Step 1 must be completed before an assessor can be assigned.

1. Open the Application Form by clicking on the APPLICATION button below.
2. Complete the Application Form.
3. Print a copy of the completed Application Form.
4. Submit the completed form on-line by clicking on the SUBMIT button at the bottom of the form.
5. Sign the printed Application Form and obtain the Principal/Director’s signature.
6. The printed form must be faxed to the number indicated on the form.

Applications are processed in the order in which they are received and can take up to three weeks to process.

Step 2: Complete Required Assessment Forms

1. Open and print the Class Profile (PDF). REQUIRED
2. Open and print the Instruction Profile (PDF). REQUIRED
3. Complete the Class Profile and the Instruction Profile forms before the assessment. Place two (2) copies of these completed forms in your school office at least one (1) hour prior to the assessor’s arrival.
   
   Candidates and mentors may download the following Pre-Observation and Post-Observation forms to use for practice for the Praxis III assessment.

Praxis III results will be e-mailed as assessments are completed, reviewed, and received by the Office of Teacher Quality.
